We are leading the way in the housing sector by managing a groundbreaking project to improve the health and wellbeing of our customers.
The collaboration with Mersey Care NHS Foundation Trust sees us integrating our social housing data - such as property types, accessibility and energy efficiency ratings - into official health records.
By doing so, we can work better with the local NHS community based Integrated Care Teams. Using our shared data the NHS can then identify patterns linking a customer’s housing conditions to various health outcomes, enabling us to provide proactive support services tailored to individuals so they can lead healthier lives.
The rationale behind this initiative is clear: people’s health is intricately linked to their living conditions.
Working with the local NHS team, we plan to run a joint pilot for 20 homes in Liverpool beginning in March 2026. We will measure the difference the project makes to those in the pilot and, if it brings benefits to customers, we will work with the NHS to see how we can expand it to cover more homes.

Once health and housing data has been integrated securely onto an NHS dashboard with tightly controlled access, the NHS will analyse the information and look at how interventions and support can be offered. The NHS will assess if Prima could have a role in helping to fix any housing issues that may be impacting on certain health conditions. The NHS Care Coordinator will ask for your permission to invite your Prima Housing Officer to meet with you and them so understand how they can help. This is called a Healthy at Home visit.
An example of how data sharing could help people is if a resident speaks to a health professional about a respiratory illness.
This may mean prioritising this tenant’s home for energy efficiency upgrades to reduce the risk of damp and mould to ease their symptoms.
Or we could be alerted when minor adaptations are needed for frail residents so they are safer in their homes.
So the NHS can identify Prima homes to consider for the project, Prima will share 4 pieces of property information with the NHS:
Unique Property Reference Number (UPRN) which all public and emergency services use.
Building type - such as a 1 bedroom ground floor flat, or a 2 bedroom house for example.
Energy efficiency rating of the building- this will be the Energy Performance Band Certificate (EPC) rating.
Adaptations installed in the property - such as a level access shower or stairlift.
Taking part in the project is voluntary and you can withdraw consent at any time. Prima will not know if your home is chosen by the NHS to be part of the project unless you agree to being involved.
Your privacy and dignity are of key importance, so the project is designed to always keep your data secure. Prima never see any health or private information. The NHS never see any personal or tenancy information.

We understand the importance of safeguarding tenants' privacy and security. Prima will not see any of your health information and will only be invited to meet with you once you have given your consent.
All data sharing will be conducted with the highest standards of confidentiality and data protection.
The data will be handled with the utmost care and respect. Prima will only share property reference number, property type, adaptations and energy rating with the NHS, we will not share any details of anyone living in the property. The NHS dont share any health details with Prima, we only become involved once you have given your consent to a Healthy at Home visit taking place
The technological approach means it is scalable nationally, and as part of this project, we are committed to developing a framework for ongoing collaboration between social housing associations and the NHS to achieve this aim so hundreds of thousands of tenants benefit.
